Resurrected after its burial by the Trump Admin last yr, a study designed to push US drinking guidance closer to a "no safe level" stance received splashy press coverage the world over today. The Alcohol Intake & Health study overseen by the Interagency Coordinating Committee on the Prevention of Underage Drinking (ICCPUD) and built to inform the new edition of the Dietary Guidelines for Americans (DGA) was published in an extensively altered form by a separate journal this wk. The NY Times unsurprisingly got the preview, headlining that "Health Risks of Alcohol Accelerate After One Drink a Day." But the paper was far from alone as outlets like the Washington Post, Philadelphia Inquirer, Reuters, NBC News, US News & World Report, The Independent, Canada's CBC and more all reprinted the study's insistence of harm at such low drinking levels. The risk of dying from an alcohol-related cause by consuming an average of one drink a day is "small," the Times acknowledged, at just 1 in 1,000. But authors insist that this risk threshold of risk is commonly used to develop guidance. So the study embraces it, explicitly urging that all adults, regardless of sex, consume no more than one drink per day. "The current results support changing the 2025-2030 US Dietary Guidelines on alcohol to recommend that current adult drinkers consume no more than 1 drink per day," the study reads. But that ship has sailed, recall.Study Broadly Rewritten, Insists on No Benefits Despite Findings to Contrary The recommendation is just one of the major ways in which the study diverges from the draft report published by ICCPUD in early 2025. At the very least, it reveals just how rough that previous draft was. The study's introduction, explanation of its methods and discussion of its import were all fully rewritten, the results completely reorganized, with some findings left out entirely. The new paper acknowledges that its analysis is based on just 16 prior articles, though the press release glosses over that, pointing to the over 7,200 papers initially considered but tossed aside.One thing that is the same about the new version: the study's actual analysis suggests some net benefits to health at less than 6-7 drinks per week. Authors couch this by saying those results are not statistically significant, emphasizing that risk levels could be higher at low levels of consumption. But they ignore that the opposite is also true: their data shows that risk levels could also be significantly lower at higher levels of consumption, including the potential for negligible risk at close to 14 drinks per week for men and as high as 11-12 drinks per week for women. Instead, they simply insist that "no protective effect of drinking was observed even at low levels."Rehash of ICCPUD vs NASEM; Vincent's Story Oversimplifies; Other Studies Complicate The publication of the study this wk opened the door to reviewing the "controversy" surrounding its creation, as the Times acknowledges. The paper and other outlets quickly rehash the drama of the dueling reports from ICCPUD and the Natl Academies (NASEM) on alcohol and health, designed to inform the DGA. Adding to the drama: the journal that published the study this wk also published an editorial by Robert Vincent, a former govt employee who oversaw ICCPUD.In his telling, he was fired as part of "targeted reduction in force" for which he seems to blame "powerful commercial interests," i.e. the alcohol industry. Yet he fails to acknowledge that on the same day he lost his job on April 1, 2025, so did thousands of other HHS employees. His insistence that he carried out his work "carefully, transparently, and in accordance with federal law" also rings hollow after ICCPUD spent many months covering up its work on the report both before and after the draft was issued, leading to a House oversight subcommittee determination that the report was "irretrievably flawed."The fixation of the Times and other outlets on this particular study is puzzling, however, as it is far from the only recently published paper on the topic. And many of their conclusions complicate the picture. For instance, while this study emphasizes cancer risk at even low levels of drinking, a separate new study only identified cancer risk at heavy drinking levels, while light drinkers experienced reductions in risk. And though this study attempts to obscure or downplay the J-shaped curve indicating protective effects of light drinking on cardiovascular health, two other new papers (one of which shares 2 authors with the ICCPUD study) confirm that link while noting other risks.
